BO‘LMACHALAR FIBRILLYATSIYASI BO‘LGAN BEMORLARDA RADIOCHASTOTALI KATETER ABLATSIYASIDAN SO‘NG SIMPATO-ADRENAL TIZIM FAOLLIGI: VEGETATIV QAYTA MOSLASHUV VA KLINIK PROGNOZ

Abstract:
Bo‘lmachalar fibrillyatsiyasi (BF) klinik amaliyotda eng ko‘p uchraydigan aritmiyalardan biri bo‘lib, insult, yurak yetishmovchiligi, gospitalizatsiya va o‘lim xavfini oshiradi. Radiochastotali kateter ablatsiyasi (RChKA), ayniqsa o‘pka venalari izolyatsiyasi (PVI), BFda ritm nazorati strategiyasining muhim bo‘g‘ini sifatida zamonaviy yo‘riqnomalarda keng qo‘llab-quvvatlanadi. Biroq RChKA natijasini faqat “elektr triggerlar”ni bartaraf etish bilan izohlab bo‘lmaydi: protsedura yurakning avtonom innervatsiyasiga (Ichki ganglionli pleksuslar, afferent-efferent reflektor yoylar) bevosita ta’sir qiladi va simpato-adrenal tizim (SAT) hamda parasimpatik komponent o‘rtasidagi vegetativ balansni qayta sozlaydi. Ushbu obzor maqolada RChKA’dan keyingi vegetativ qayta moslashuv mexanizmlari, uni baholash markerlari (yurak urish tezligi oralig‘i o‘zgaruvchanligi, yurak urish tezligi dinamikasi, “vagal javob”, barorefleks ko‘rsatkichlari va ayrim biokimyoviy markerlar) hamda ularning BF qaytalanishi va klinik natijalar bilan bog‘liqligi tizimli ravishda yoritiladi. Shuningdek, zamonaviy konsensus hujjatlarida ablatsiyadan keyingi monitoring, qaytalanish endpointlari bo‘yicha qarashlar, vegetativ markerlarni amaliy prognozlashga joriy etishdagi cheklovlar va istiqbolli tadqiqot yo‘nalishlari muhokama qilinadi. BF patogenezida vegetativ omilni chuqurroq tushunish va standartlashtirilgan baholash protokollarini ishlab chiqish RChKA natijasini individual prognozlash, qaytalanish xavfini erta aniqlash va kuzatuv taktikalarini optimallashtirishda ahamiyatli bo‘lishi mumkin.
