Cardiovascular diseases (CVDs) are globally recognized as one of the leading threats to human health, accounting for more than 17 million deaths annually worldwide (World Health Organization, 2024). In Uzbekistan, these diseases significantly impact public health, with the proportion of chronically ill patients steadily increasing. This situation necessitates long-term follow-up, individualized approaches, and strengthening of home-based patronage services. Improving primary health care (PHC) delivery ensures the development of a systematic and integrated model of continuous medical supervision.
