Abstract

Vitiligo — melanotsitlarning immun-mediator mexanizm orqali nobud bo‘lishi natijasida teri va shilliq qavatlarda depigmentlangan (sutdek oq) dog‘lar paydo bo‘lishi bilan kechadigan surunkali autoimmun dermatoz hisoblanadi. Kasallik dunyo aholisining taxminan 0,5–2 foizida uchraydi, istalgan yoshda boshlanishi mumkin, biroq ko‘pincha 10–30 yoshlarda namoyon bo‘ladi va jinsga bog‘liq emas. Klinik kechishi bo‘yicha segmentar bo‘lmagan (generalizatsiyalangan) va segmentar shakllar farqlanadi. So‘nggi yillardagi tadqiqotlar kasallik patogenezida interferon-gamma (IFN-γ)–JAK–STAT signal yo‘lining markaziy o‘rin tutishini ko‘rsatdi: bu yo‘l sitotoksik CD8⁺ T-limfotsitlarni jalb qilib, CXCL10 kabi xemokinlar ishlab chiqarilishi va melanotsitlar apoptozini kuchaytiradi [1]. Etiopatogenezda autoimmun mexanizm bilan birga oksidlovchi stress, genetik moyillik va neyrogen omillar ham ishtirok etadi; vitiligo ko‘pincha qalqonsimon bez patologiyasi va boshqa autoimmun kasalliklar bilan birga uchraydi. Ko‘rinadigan sohalarda joylashganligi sababli kasallik bemorning ruhiy holati, hayot sifati va ijtimoiy moslashuviga sezilarli salbiy ta’sir ko‘rsatadi
